The Demat Account modification form is given in the attachment to this webpage. Please download it, fill both pages and send it to info@goelasf.in and cc to goelasfconnect@gmail.com.
Please also attach the following documents for respective modification:

Bank Modification: latest Bank Statement PDF
Address Modification: Aadhar Card
Name Modification: Aadhar and PAN both along with latest Bank Statement PDF
